Job Description

We are seeking a skilled Intake Home Infusion Pharmacist to join our team in West Chester, OH. As a nationwide specialty pharmacy, we focus on providing services for patients with chronic and rare illnesses that require complex care.

In this role, you will be responsible for performing various distributive and clinical pharmacy activities to ensure the safe and effective delivery of home infusion therapies.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Ensure adherence to company policies and procedures.
• Create and implement policies and procedures for safe handling of infusion therapy throughout the pharmacy process.
• Collaborate with nursing, customer service, and distribution teams to facilitate new referrals and admissions.
• Review and verify new patient orders, documenting them accordingly.
• Assist in planning and implementing patient care, working closely with physicians and healthcare professionals.
• Conduct assessments to identify drug-related problems and needs.
• Set therapeutic objectives and parameters for drug therapy monitoring.
• Monitor and interpret outcomes of drug therapy.
• Effectively communicate therapeutic drug monitoring results to appropriate individuals.
• Program and manage infusion control devices.
• Provide consultation and answer inquiries regarding drugs and their use.
• Maintain accurate documentation of clinical activities and communication with patients and healthcare professionals.
• Prepare progress reports for referring physicians on a regular basis.
• Process and verify medications, supplies, and equipment prior to dispensing.
• Stay updated on pharmacotherapy by reviewing literature and attending relevant events.
• Participate in on-call rotation.
• Assist with marketing support functions as needed.
• Contribute to orientation and training of new personnel.
• Participate in departmental quality assurance activities.
• Attend scheduled nursing department meetings.
• Maintain pharmacy records in compliance with regulations.
• Handle customer complaints according to company policies.
• Educate referral sources on services and benefits.
• Submit required reports in a timely manner.
• Maintain current files and records as instructed.
• Collaborate with senior management to improve efficiency and product knowledge.
• Assist with special assignments and market tests.
• Deliver high-quality services to home-care patients and referral sources.
• Attend conventions, trade shows, and in-services as directed by management.
• Represent the company professionally at all times.
• Maintain effective working relationships with colleagues.
• Comply with company's policies and procedures.
• Perform any other assigned duties and responsibilities.

QUALIFICATION REQUIREMENTS

EDUCATION AND/OR EXPERIENCE

• Hold a Bachelor of Science in Pharmacy (Doctor of Pharmacy or a Master of Science degree in Pharmacy is desirable) from an accredited college of pharmacy.
• Currently licensed as a registered pharmacist in the state of employment.
• Have a minimum of 2 years of experience in specialty pharmacy or ICU/critical care (experience with plasma-derived therapy is highly preferred).
• Demonstrate the ability to evaluate and monitor drug therapy for home care patients.

Physical Demands

The physical demands of this job include talking, standing, walking, sitting, climbing stairs, using hands to handle objects, tools, or controls, and reaching with hands and arms. The employee may also be required to lift and move up to 20 pounds. Clear vision and the ability to adjust focus are necessary.

Work Environment

This job primarily takes place in a clerical office setting. The employee will use standard office equipment and may need to travel by automobile, exposed to different weather conditions.
