We are seeking dedicated individuals to join our team as Rehabilitation Specialists. As a member of our physical therapy staff, you will play a key role in ensuring the health and well-being of our residents by providing comprehensive and high-quality therapy services. A valid state-specific Physical Therapy license is required for this position.
Responsibilities:

Initiate treatment plans based on physician's orders and maintain regular contact with physicians.

Keep accurate written records of evaluations, progress notes, and discharge summaries for residents in the physical therapy program.

Collaborate with other healthcare staff to address the special needs of residents and enhance their rehabilitation or maintenance of function.

Contribute to Resident Care Planning meetings and attend them in the absence of the Director of Physical Therapy.

Maintain statistical records of residents' treatments.

Assist with in-service training programs as needed.

Update and maintain resident's plan of care in the Inter-disciplinary Kardex.

Submit requisitions for supplies required in the treatment of residents to the Director of Physical Therapy.

Assume all responsibilities of the Director of Physical Therapy in their absence, including supervising Physical Therapy Aides and ensuring all necessary documentation is completed.
Qualifications:

Graduation from an approved school of physical therapy is required.

A valid state license to practice physical therapy is required.

Preference will be given to candidates with a minimum of two years' experience in physical therapy.

Strong interest in providing physical therapy treatment to the geriatric and subacute populations is necessary.

Must possess the ability to provide direction and guidance to others.
