MAIN FUNCTION:

1. Collaborate with interdisciplinary treatment team to plan and implement specific treatment programs for individual patients according to the principles and practices of Physical Therapy.
2. Accountable to the Clinical Coordinator of Rehab Services.

DUTIES AND RESPONSIBILITIES:

1. Evaluate patient's physical condition and needs through interviews and assessments.

2. Develop and implement a program of corrective exercise and treatment for assigned patients.

3. Provide instructions on proper equipment usage, body positioning, and use of assistive devices.

4. Review and modify treatment plans based on patient's progress and consult with physicians when needed.

5. Maintain accurate documentation of evaluations, treatment plans, progress reports, and discharge summaries.

6. Collaborate with nursing and interdisciplinary team members to ensure coordination of patient care.

7. Provide guidance to assisting staff members and maintain department policies, procedures, and safety protocols.

8. Assist in the care and maintenance of department equipment and supplies.

9. Attend meetings and participate on committees as required.

10. Transport patients to and from the PT Department as needed.

11. Assume the duties and responsibilities of Senior Therapists in their absence.

12. Continuously enhance professional growth and development through educational programs and literature.

13. Provide age-appropriate care to patients across various age groups.

14. Ensure customer satisfaction by interacting with patients in a friendly and caring manner.

EDUCATION:

• Must be licensed or eligible to obtain a license to practice physical therapy in NYS at the time of hire.

EXPERIENCE:

• Prior experience in a hospital setting preferred.

CARDIOPULMONARY RESUSCITATION (CPR) REQUIREMENTS:

• Basic Life Support (BLS) certification required within 90 days of hire date.

PHYSICAL DEMANDS:

• Requires moderate physical effort, including lifting, pushing, and pulling.
• Must be able to lift at least 30 pounds independently and up to 150 pounds with assistance.

EXPOSURE CATEGORY:

• Category II. No exposure to blood, body fluids, or tissues. May perform unplanned Category I tasks.

INFECTION CONTROL:

• Adheres to universal precautions when engaging in patient care.
