Physical Therapist / Full-time - Home Health

Important Note: Please ensure that you have prior experience in the home health industry before applying for this position.

We are open to negotiation on compensation!

Responsibilities include, but are not limited to:
Administering treatment to homebound patients as prescribed by the attending physician, focusing on pain relief, function restoration, and maximizing performance through various therapeutic techniques. Documenting all treatments and observations in the electronic medical record and promptly notifying the physician of any significant changes in the patient's condition.

Expertise in PT Home Health

Proficiency in computer usage and the ability to maintain basic records in English.

Utilizing verbal and written communication techniques to provide patient and family education that is culturally, ethnically, and personally sensitive.

Collaborating with patients and their families to teach appropriate self-care techniques.

Adhering to Agency policy by submitting documentation in a timely manner.

Staying updated on federal, state, and agency regulations regarding patient care, including incident management and reporting.

Meeting all regulatory requirements.

Participating in mandated activities such as in-services, Quality Assurance and Performance Improvement (QAPI), and staff meetings.

EDUCATION:

Licensed as a Physical Therapist in the State

QUALIFICATIONS:

· Hold a valid and unrestricted State professional license.

· Possess at least one year of experience as a physical therapist in home care and demonstrate competence in comprehensive assessment and OASIS-C.

· Ability to make informed clinical judgments.

· Skilled in assessing patient needs and creating customized care plans.

· Proficient in clinical skills.

· Excellent written and verbal communication skills, with fluency in English.

· Own a reliable vehicle that can be used for work, possess a valid driver’s license, maintain a clean driving record, and provide proof of insurance.

· Proficiency in using personal computers, including email, clinical software, word processing, and spreadsheets.
