Physical Therapist / Full-time - Home Health

Please note: This position requires prior experience in the home health industry.

Responsibilities:
Serving as a Physical Therapist, providing ordered treatment to homebound patients to alleviate pain, restore function, and maintain optimal performance using a range of therapeutic techniques. Accurately documenting all treatments and changes in medical records and promptly reporting any significant changes in the patient's condition to the attending physician.

PT Home Health Proficiency

▪ Ability to use computers and maintain basic records in English.

▪ Utilizing various verbal and written communication techniques to educate patients and their families, considering their cultural, ethnic, or personal preferences.

▪ Collaborating with patients and their families to teach appropriate self-care techniques.

▪ Timely submission of documentation in adherence to Agency policy.

▪ Up-to-date knowledge of federal, state, and agency regulations regarding patient care, including incident management and reporting.

▪ Compliance with regulatory requirements.

▪ Active involvement in agency's mandated activities such as in-services, Quality Assurance and Performance Improvement (QAPI), and staff meetings.

EDUCATION:

Valid Physical Therapist license in the state

REQUIREMENTS:

· Current and unrestricted professional license in the state.

· Minimum of one year experience as a home care physical therapist, with competency in comprehensive assessment and OASIS-C preferred.

· Demonstrated ability to make sound clinical judgments.

· Proficient in assessing and documenting patient needs and creating personalized care plans.

· Proficiency in clinical skills.

· Excellent written and verbal communication skills with proficiency in English.

· Possession of a reliable vehicle for work, a valid driver's license, a clean driving record, and proof of insurance.

· Proficiency in using personal computers and software such as email, clinical applications, word processing, and spreadsheets.
