Physical Therapist - Home Health in Suffolk County
Responsibilities
- Delivers high-quality therapy services in accordance with the applicable Standards of Practice
- Conducts comprehensive evaluations of patients and creates individualized treatment plans with both short and long-term goals
- Regularly assesses and reports the patient's progress, reactions to treatments, and any changes in their condition, making necessary modifications to the treatment plan
- Provides instruction to patients, caregivers, and healthcare team members on techniques to enhance and correct disabilities
- Maintains regular communication with physicians and other healthcare professionals
- Develops realistic and achievable long-term goals for patient discharge, adjusting as necessary based on changes in condition
- Prepares patients and their caregivers for care after discharge
- Reports any significant patient, family, or physician issues to the Clinical Director
Requirements & Qualifications
- Must possess a valid state licensure, CPR certification, driver's license, and automobile liability insurance
- Demonstrates thorough knowledge of professional theory, practice, techniques, principles, and equipment
- Exceptional oral and written communication skills, with the ability to effectively document patient care
- Capable of establishing and maintaining strong relationships with patients, healthcare professionals, physicians, and the general public
- A minimum of one (1) year of experience is required
