  • Delivers high-quality therapy services according to the applicable Standards of Practice
  • Conducts patient evaluations and develops personalized treatment plans with short and long-term goals
  • Assesses patient progress, monitors reactions to treatments, and communicates any changes to the responsible physician; adjusts treatment plans as necessary
  • Provides instructions to patients, caregivers, and other healthcare team members on methods to improve and address disabilities
  • Maintains regular communication with physicians and other healthcare professionals
  • Develops realistic and achievable long-term goals for patient discharge, which may be adapted as conditions change
  • Prepares patients and caregivers for post-discharge care
  • Reports any significant patient, family, or physician issues to the Clinical Director

Requirements & Qualifications

  • Must possess current state licensure, CPR certification, driver's license, and automobile liability insurance
  • Demonstrates comprehensive knowledge of professional theory, practice, techniques, principles, and equipment
  • Exhibits effective oral and written communication skills and the ability to document patient care accurately
  • Establishes and maintains productive relationships with patients, healthcare professionals, physicians, and the general public
  • Minimum of one (1) year of experience is required
