Responsibilities:
- Delivers high-quality therapy services in accordance with the established Standards of Practice.
- Conducts patient evaluations and creates personalized treatment plans with both short-term and long-term goals.
- Monitors and reports the patient's progress, treatment reactions, and any changes in their condition to the attending physician; adjusts treatment plans as necessary.
- Teaches patients, caregivers, and healthcare team members about techniques to improve and address disabilities.
- Maintains regular communication with physicians and other members of the healthcare team.
- Develops realistic and achievable long-term goals for patient discharge which can be modified as the patient's condition changes.
- Prepares patients and caregivers for post-discharge care.
- Notifies the Clinical Director of any significant patient, family, or physician related matters.
Requirements & Qualifications:
- Active state licensure, CPR certification, driver's license, and automobile liability insurance.
- In-depth knowledge of professional theory, techniques, equipment, and principles.
- Excellent oral and written communication skills with the ability to document patient care effectively.
- Ability to establish and maintain positive relationships with patients, healthcare professionals, physicians, and the general public.
- A minimum of one (1) year of experience is required.
