Key Qualifications:
• Bachelor's degree or higher in Science in Allied Health Professions in Physical Therapy from an accredited program is a requirement.
• Must hold a current license as a Physical Therapist in the State of Ohio.
• Two years of community/home health experience as a physical therapist is preferred.
• Current CPR (AHA BLS) certificate is required.
• Must possess a valid Ohio driver's license and maintain motor vehicle insurance.
Hours and Compensation:
• Full-time position, working from 8am-4:30pm.
• Salary range is between $70K-$102K, depending on experience. We also offer a comprehensive benefits package and a sign-on bonus of $25K.
Responsibilities:
• Deliver physical therapy services to patients receiving home health care. Oversee the work of Physical Therapy Assistants and ensure compliance with standard practices in a home health setting.
• Follow the physician's written plan of care and provide therapeutic evaluations, rehabilitation assessments, and functional status assessments. Evaluate the patient's home environment.
• Assess muscle strength, mobility, gait, and range of motion for potential rehabilitation.
• Administer physical therapy treatments and educate patients and their families/caregivers on the proper use and care of therapeutic appliances.
• Identify priority needs for physical therapy and report any changes in the patient's condition to the physician.
• Complete medication reconciliation for therapy-only patients as needed. Ensure timely completion of OASIS charting.
• Initiate the physical therapy program and train other personnel or family/caregivers involved in the patient's care. Participate in case conferences.
• Ensure all documentation for each visit is completed within 72 hours. Provide training to patients on the use of prosthetic devices, if required.
• Identify additional home health services needed by the patient and make referrals as necessary.
• Prepare and submit clinical and progress summaries based on goal attainment. Contribute to discharge planning.
