Responsibilities
- Delivers high-quality therapy services according to the applicable Standards of Practice
- Conducts patient evaluations and develops personalized treatment plans with short and long-term goals
- Assesses patient progress, monitors reactions to treatments, and communicates any changes to the responsible physician; adjusts treatment plans as necessary
- Provides instructions to patients, caregivers, and other healthcare team members on methods to improve and address disabilities
- Maintains regular communication with physicians and other healthcare professionals
- Develops realistic and achievable long-term goals for patient discharge, which may be adapted as conditions change
- Prepares patients and caregivers for post-discharge care
- Reports any significant patient, family, or physician issues to the Clinical Director
Requirements & Qualifications
- Must possess current state licensure, CPR certification, driver's license, and automobile liability insurance
- Demonstrates comprehensive knowledge of professional theory, practice, techniques, principles, and equipment
- Exhibits effective oral and written communication skills and the ability to document patient care accurately
- Establishes and maintains productive relationships with patients, healthcare professionals, physicians, and the general public
- Minimum of one (1) year of experience is required
