Role and Responsibilities:
  • As a qualified professional, you will be responsible for directing, supervising, evaluating, and providing physical therapy services to adult patients in their homes, based on physician’s orders.

Essential Functions:

  • Conduct diagnostic tests and measures to evaluate the patient's muscle, nerve, joint, and functional abilities.
  • Assess the patient's condition during the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care, and determine the types and frequencies of treatment.
  • Communicate with the physician about any necessary modifications to the plan of care.
  • Document clinical notes on each visit and keep all records and summaries up-to-date.
  • Provide treatment to address impairments and functional limitations, aiming for maximum functional independence.
  • Instruct and educate patients and caregivers on the use of prosthetic, orthotic, and durable medical equipment.
  • Educate patients and caregivers about the treatment plan and goals.
  • Participate in physical therapy staff meetings and case conferences.
  • Collaborate in the development and revision of the physician's Plan of Treatment.
  • Ensure appropriate safety practices and equipment maintenance.
  • Perform any other assigned duties.

Education Requirements:

  • Graduate from an accredited Physical Therapy program.

Qualifications and Requirements:

  • Hold a current, valid Physical Therapy license issued by NYS Department of Education.
  • One year of licensed physical therapist experience.

Work Environment:

  • The work location will primarily be in non-office settings, and there may be exposure to outdoor conditions.

The working conditions are classified as medium work:

  • Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly, including lifting, carrying, pushing, pulling, or moving objects and sitting and standing.

Physical Requirements:

On a daily basis, the Physical Therapist is required to:
