Role and Responsibilities:
  • You will be responsible for delivering high-quality physical therapy services to adult patients in their homes, as prescribed by physicians.

Essential Functions:

  • Perform diagnostic tests to evaluate patients' muscle, nerve, joint, and functional abilities.
  • Conduct initial evaluations and periodic re-evaluations to assess the patients' condition and create a comprehensive plan of care, including treatment types and frequencies.
  • Consult with physicians and adjust the treatment plan as necessary.
  • Maintain accurate and up-to-date clinical records and patient summaries.
  • Provide treatment to address impairments and functional limitations, with the goal of improving the patients' independence.
  • Educate and instruct patients and caregivers on the use of assistive devices and medical equipment.
  • Ensure patient understanding of the treatment plan and goals.
  • Attend physical therapy staff meetings and participate in case conferences.
  • Collaborate in the development and revision of physicians' Plans of Treatment.
  • Maintain a safe work environment and proper equipment maintenance.
  • Perform any other assigned duties.

Education Requirements:

  • Graduate of an accredited Physical Therapy program.

Qualifications and Requirements:

  • Current and valid Physical Therapy license issued by the NYS Department of Education.
  • At least one year of practical experience as a licensed physical therapist.

Work Environment:

  • This position primarily involves visiting patients' homes and may require exposure to outdoor conditions.

The working conditions are classified as medium work:

  • Medium work - Involves lifting up to 50 pounds occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or otherwise move objects, including the human body. This work also includes sitting and standing.
