Responsibilities:
- Provide physical therapy services to adult patients in their homes, following physician's orders
Key Functions:
- Evaluate patient using diagnostic muscle, nerve, joint, and functional ability tests and measures
- Assess the patient's condition and develop a written problem list and plan of care
- Recommend modifications to the treatment plan as needed
- Document clinical notes and keep records up-to-date
- Treat patients to address impairments and functional limitations, aiming for maximum functional independence
- Instruct and educate patients and caregivers on the use of medical equipment
- Attend physical therapy staff meetings and case conferences
- Participate in the development and revision of the physician's Plan of Treatment
- Ensure safety practices and equipment maintenance
- Perform other duties as assigned
Education Requirements:
- Graduate of an accredited Physical Therapy program
Qualifications and Requirements:
- Current and valid Physical Therapy license issued by NYS Department of Education
- Minimum of one year of practical experience as a licensed physical therapist
Work Environment:
- The Physical Therapist primarily works in non-office settings and may be exposed to outdoor conditions
The working conditions are classified as medium work:
- Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
