Responsibilities:
  • Provide physical therapy services to adult patients in their homes, following physician's orders

Key Functions:

  • Evaluate patient using diagnostic muscle, nerve, joint, and functional ability tests and measures
  • Assess the patient's condition and develop a written problem list and plan of care
  • Recommend modifications to the treatment plan as needed
  • Document clinical notes and keep records up-to-date
  • Treat patients to address impairments and functional limitations, aiming for maximum functional independence
  • Instruct and educate patients and caregivers on the use of medical equipment
  • Attend physical therapy staff meetings and case conferences
  • Participate in the development and revision of the physician's Plan of Treatment
  • Ensure safety practices and equipment maintenance
  • Perform other duties as assigned

Education Requirements:

  • Graduate of an accredited Physical Therapy program

Qualifications and Requirements:

  • Current and valid Physical Therapy license issued by NYS Department of Education
  • Minimum of one year of practical experience as a licensed physical therapist

Work Environment:

  • The Physical Therapist primarily works in non-office settings and may be exposed to outdoor conditions

The working conditions are classified as medium work:

  • Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
