• Supportive Leadership
- As a skilled professional, you will provide physical therapy services to adult patients in their homes as prescribed by their physicians.
Essential Functions:
- Conduct diagnostic tests and assessments to evaluate the patient's muscle, nerve, joint, and functional abilities.
- Develop and update a written problem list and care plan, including treatment types and frequencies, based on initial evaluations and periodic re-evaluations.
- Consult with physicians and make recommendations for modifications to the care plan, as needed.
- Maintain up-to-date and accurate records of all patient interactions and treatments.
- Administer treatments to address impairments and improve functional independence.
- Provide instruction and guidance to patients and their caregivers regarding the use of prosthetic, orthotic, and durable medical equipment.
- Educate patients and caregivers on treatment plans and goals.
- Participate in physical therapy staff meetings and case conferences.
- Collaborate in the development and revision of physician's treatment plans.
- Ensure safety practices and equipment maintenance are in adherence to guidelines.
- Perform any other assigned duties.
Education Requirements:
- Graduate of an accredited Physical Therapy program.
Qualifications and Requirements:
- Valid Physical Therapy license issued by NYS Department of Education.
- At least one year of practical experience as a licensed physical therapist.
Work Environment:
- The Physical Therapist primarily works in non-office settings, including possible exposure to outdoor conditions.

- Medium work - Includes regularly exerting up to 50 pounds of force to lift, carry, push, pull, or move objects, including the human body. Involves sitting and standing.
