- As a qualified professional, your role will involve directing, supervising, evaluating, and providing physical therapy services to adult patients in their homes, as directed by physicians.
Essential Functions:
- Conduct diagnostic tests and assessments to evaluate muscle, nerve, joint, and functional ability of patients.
- Develop and periodically revise written problem lists, care plans, and treatment schedules based on evaluations.
- Communicate with physicians to recommend modifications in care plans, as necessary.
- Maintain accurate and up-to-date clinical records and notes.
- Address patient impairments and limitations, aiming for maximum functional independence.
- Educate patients and caregivers on the use of assistive devices and equipment.
- Keep patients and caregivers informed about treatment plans and goals.
- Participate in physical therapy staff meetings and case conferences.
- Collaborate in the development and revision of physicians' plans of treatment.
- Ensure adherence to safety practices and equipment maintenance.
- Perform any other assigned duties.
Education Requirements:
- Graduation from an accredited Physical Therapy program
Qualifications and Requirements:
- Current, valid Physical Therapy license issued by the NYS Department of Education
- At least one year of practical experience as a licensed physical therapist
Work Environment:
- The work primarily takes place in non-office settings, with exposure to outdoor conditions.
The working conditions are classified as medium work:
- Medium work - this involves exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or move objects, including the human body. It also involves sitting and standing.
