Role and Responsibilities:
  • As a qualified professional, you will be responsible for directing, supervising, evaluating, and providing physical therapy services to adult patients in their homes, following physician's orders.

Essential Functions:

  • Conduct diagnostic tests and assessments to evaluate patients' muscle, nerve, joint, and functional abilities.
  • Develop written problem lists and care plans, including recommended treatments and frequencies, based on initial evaluations and perform periodic re-evaluations.
  • Communicate with physicians and recommend modifications to the care plan, as needed.
  • Maintain accurate and up-to-date clinical notes and records in the medical files.
  • Administer treatments to address impairments and functional limitations, with the goal of achieving maximum functional independence.
  • Educate and instruct patients and caregivers on the use of prosthetic, orthotic, and durable medical equipment.
  • Ensure that patients and caregivers understand the treatment plan and goals.
  • Participate in physical therapy staff meetings, case conferences, and the development of the physician's Plan of Treatment.
  • Maintain safety practices and equipment maintenance.
  • Perform other assigned duties as required.

Education Requirements:

  • Graduate of an accredited Physical Therapy program.

Qualifications and Requirements:

  • Current and valid Physical Therapy license issued by the NYS Department of Education.
  • Minimum of one year of licensed physical therapy experience.

Work Environment:

  • The work primarily takes place in non-office settings, including exposure to outdoor conditions.

The working conditions are classified as medium work:

  • Medium work - Involves exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or move objects, including the human body. Involves sitting and standing.
