- As a qualified professional, your role involves directing, supervising, evaluating, and providing physical therapy services to adult patients in their homes as prescribed by physicians.
Essential Functions:
- Evaluate patients using diagnostic tests and measures for muscle, nerve, joint, and functional ability.
- Conduct initial assessments and perform periodic re-evaluations, developing written problem lists and individualized care plans.
- Recommend modifications to the care plan as required by the physician.
- Document clinical notes on the day of the visit and keep all records up-to-date.
- Treat patients to address impairments and functional limitations, aiming for maximum functional independence.
- Instruct and advise patients and their caregivers on the usage of prosthetic, orthotic, and durable medical equipment.
- Ensure patients and caregivers understand the treatment plan and goals.
- Attend physical therapy staff meetings and case conferences.
- Contribute to the development and revision of the physician's Plan of Treatment.
- Maintain equipment and practice appropriate safety measures.
- Complete other assigned duties as needed.
Education Requirements:
- Graduate of an accredited Physical Therapy program.
Qualifications and Requirements:
- Hold a current, valid Physical Therapy license issued by the NYS Department of Education.
- Have at least one year of practical experience as a licensed physical therapist.
Work Environment:
- The Physical Therapist primarily works in a non-office setting and may be exposed to outdoor conditions.
Working Conditions:
- This role is classified as medium work, involving occasional exertion of up to 50 pounds of force and frequent or constant movement of objects.
