• As a qualified professional, your role involves directing, supervising, evaluating, and providing physical therapy services to adult patients in their homes as prescribed by physicians.

Essential Functions:

• Evaluate patients using diagnostic tests and measures for muscle, nerve, joint, and functional ability.
• Conduct initial assessments and perform periodic re-evaluations, developing written problem lists and individualized care plans.
• Recommend modifications to the care plan as required by the physician.
• Document clinical notes on the day of the visit and keep all records up-to-date.
• Treat patients to address impairments and functional limitations, aiming for maximum functional independence.
• Instruct and advise patients and their caregivers on the usage of prosthetic, orthotic, and durable medical equipment.
• Ensure patients and caregivers understand the treatment plan and goals.
• Attend physical therapy staff meetings and case conferences.
• Contribute to the development and revision of the physician's Plan of Treatment.
• Maintain equipment and practice appropriate safety measures.
• Complete other assigned duties as needed.

Education Requirements:

• Graduate of an accredited Physical Therapy program.

Qualifications and Requirements:

• Hold a current, valid Physical Therapy license issued by the NYS Department of Education.
• Have at least one year of practical experience as a licensed physical therapist.

Work Environment:

• The Physical Therapist primarily works in a non-office setting and may be exposed to outdoor conditions.

Working Conditions:

• This role is classified as medium work, involving occasional exertion of up to 50 pounds of force and frequent or constant movement of objects.
