Role and Responsibilities:
  • As a Physical Therapist, you will evaluate, supervise, and provide physical therapy services to adult patients in their homes, following physician's orders.

Essential Functions:

  • Perform diagnostic tests to evaluate the patient's muscle, nerve, joint, and functional abilities.
  • Assess the patient's condition and develop a written problem list and plan of care, including treatment types and frequencies.
  • Consult with the physician to recommend modifications to the plan of care if needed.
  • Keep accurate and up-to-date clinical notes and records.
  • Treat patients to address impairments and improve functional independence.
  • Educate and instruct patients and caregivers on the use of medical equipment.
  • Communicate treatment plan and goals to patients and caregivers.
  • Attend staff meetings and case conferences.
  • Contribute to the development and revision of the physician's Plan of Treatment.
  • Ensure safety practices and equipment maintenance.
  • Perform other assigned duties as necessary.

Education Requirements:

  • Graduate of an accredited Physical Therapy program.

Qualifications and Requirements:

  • Current, valid Physical Therapy license issued by NYS Department of Education.
  • Minimum of one year of experience as a licensed physical therapist.

Work Environment:

  • The Physical Therapist primarily works in non-office settings, including possible exposure to outdoor conditions.

The working conditions involve medium work:

  • Medium work - Involves occasional exertion of up to 50 pounds of force and/or frequent or constant exertion of a negligible amount of force to move objects, including the human body. This work requires sitting and standing.

Physical Requirements:

The Physical Therapist should be able to perform the following physical tasks on a daily basis:

  • Stand
  • Sit
  • Hear
  • Walk
  • Talk
  • Stoop or kneel
  • Perform repetitive motion
