• Join our team as a qualified Physical Therapist specializing in home visits. You will evaluate, supervise, and provide physical therapy services to adult patients in their homes, following physician’s orders.

Essential Functions:

• Conduct diagnostic tests and assessments to evaluate patients' muscle, nerve, joint, and functional abilities.
• Create and update patient assessment reports, problem lists, and personalized care plans.
• Collaborate with physicians to modify treatment plans as needed.
• Maintain accurate and up-to-date clinical notes and records.
• Treat patients to enhance their physical impairments and functional independence.
• Instruct and educate patients and caregivers on the use of medical equipment and assistive devices.
• Communicate treatment plans and progress to patients and caregivers.
• Attend physical therapy staff meetings and case conferences.
• Assist in the development and revision of physician's treatment plans.
• Ensure safety practices and equipment maintenance.
• Perform any other assigned duties.

Education Requirements:

• Graduated from an accredited Physical Therapy program.

Qualifications and Requirements:

• Hold a current and valid Physical Therapy license issued by the NYS Department of Education.
• Have at least one year of practical experience as a licensed physical therapist.

Work Environment:

• As a Physical Therapist, you will primarily perform your duties during home visits and may occasionally work outdoors.

The working conditions fall under medium work classification:

• Medium work - involves exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or move objects, including the human body. This work involves both sitting and standing.
