About the Role:

  1. As a Physical Therapist, you will be responsible for delivering comprehensive physical therapy services including assessments, treatment planning, and therapeutic interventions within an interdisciplinary environment. This position requires adherence to professional practices, ethical standards, and active involvement in program development, quality improvement, problem-solving, and enhanced productivity within a collaborative setting.


  • Minimum of a four-year Bachelor's degree in Physical Therapy from an accredited program (or its foreign equivalent).
  • Valid state licensure for practice is essential.
  • Current BLS Certification is mandatory.


  • Prior experience and education in providing rehabilitation services to individuals with disabilities or injuries across different age groups.

Key Responsibilities:

1. Display professional conduct and maintain a positive demeanor when interacting with patients, families, colleagues, and other stakeholders.

2. Administer initial assessments using appropriate tools and adapt equipment/materials to match the patient's age and developmental stage. Document assessment outcomes in the medical records as per established policies and procedures.

3. Develop personalized treatment plans with clear and measurable short-term and long-term goals that address specific functional deficits. Select interventions based on assessment results, ensuring suitability to the patient's age, cognitive abilities, and developmental level. Administer prescribed medication as needed.

4. Evaluate patient response to treatment, revise treatment plans accordingly, and keep patients and their families informed of progress, program planning, and discharge plans throughout the rehabilitation process.

5. Document the course of patient care, including progress made, justification for continued skilled intervention, and any clinical recommendations provided to referring providers. Complete daily progress notes in adherence to policies and procedures. Ensure accurate capture of charges related to treatment given. Prepare discharge summaries in line with policy guidelines.

6. Provide patient and family education to enhance functional independence. Involve family members in the treatment, education, and discharge planning process, ensuring a safe transition home while meeting ongoing care needs. Measure effectiveness of education through peer input, family feedback, patient satisfaction surveys, and quality metrics. Adapt educational methods to suit patient and family educational requirements. Collaborate with a multidisciplinary team to determine appropriate next levels of care during discharge planning.

7. Participate in continuing education and professional development activities, including departmental in-service sessions and external growth opportunities. Share acquired knowledge with the team.

8. Adhere to federal and state guidelines, professional codes, employee handbooks, unit policies and procedures, and organizational policies.

9. Meet established productivity standards with fiscal responsibility.

Additional Responsibilities:

  • Complete all assigned duties and handle all equipment within the Physical Therapy Department as detailed in the Competency Checklist.
