Job Title: Outpatient Physical Therapist with Sign-On Bonus

Position Summary:

  • Primarily work in an outpatient setting with a focus on providing one-on-one care to patients.
  • Responsible for delivering a wide range of physical therapy services, including assessments, treatment planning, and therapeutic interventions, in an interdisciplinary setting. Maintain high ethical standards and contribute to program development, quality improvement, problem-solving, and productivity enhancement.

Education, License & Certifications:

  • Minimum requirement of a bachelor's degree from an accredited four-year program (or foreign equivalent) in physical therapy.
  • Current licensure in the state of practice is mandatory.
  • Valid BLS Certification is required.

Experience:

  • Experience and education in assisting patients with disabilities or injuries throughout their lifespan, helping them in their rehabilitation journey.

Essential Functions:

1. Demonstrate professionalism while communicating and interacting with patients, families, colleagues, and others.

2. Perform initial assessments using appropriate tools and equipment. Document assessment results accurately and effectively, adhering to organizational policies and procedures.

3. Develop personalized treatment plans with measurable, time-bound goals that address functional deficits. Select appropriate interventions based on assessment outcomes, considering patients' age, cognitive abilities, and development level. Administer medication as prescribed, if necessary.

4. Evaluate patients' response to treatment. Revise treatment plans and communicate progress, program planning, and discharge plans with patients/families throughout the rehabilitation process.

5. Document patient care progress, including treatment outcomes and recommendations for further clinical intervention or discharge. Adhere to policy and procedure for daily progress notes, capture of charges, and completion of discharge summaries.

6. Provide patient and family education on tasks and resources that promote continued functional independence. Involve family members in the treatment, education, and discharge planning process. Participate in multidisciplinary team discussions on the next level of care.

7. Engage in continuing education and professional development activities. Attend and present at in-service sessions and external professional growth events as required.

8. Adhere to federal and state guidelines, professional code of conduct, organizational policies, and specific facility policies. Maintain organizational productivity standards in a cost-effective manner.

Other Duties:

  • Perform all equipment and duties within the Physical Therapy Department as listed on the Competency Checklist.
