Are you a skilled and dedicated Physical Rehabilitation Specialist looking for an exciting full-time opportunity in Phoenix? In this role, you will have the chance to provide exceptional therapeutic services to patients, allowing them to receive care in the comfort of their own homes and greatly improving their quality of life.

Primary Responsibilities:

• Conduct comprehensive assessments and diagnostic tests in order to create effective care plans based on physician directives.
• Complete initial evaluations for clients within 48 hours of referral, ensuring accurate documentation and adherence to agency policies.
• Collaborate with an interdisciplinary team to effectively communicate plans, progress, and modifications to physicians and case managers.
• Educate and counsel patients and their families on physical therapy needs and available community resources.
• Contribute to the discharge planning process and participate in quality improvement initiatives, utilizing tools like OASIS for assessment and outcome analysis.
• Maintain a safe environment for both clients and coworkers by following safety and infection control practices.
• Supervise and evaluate members of the home care team, including therapy assistants and aides, according to agency guidelines.

Qualifications:

• Graduate of a recognized Physical Therapy program accredited by relevant medical and educational authorities.
• Current state licensure/registration as a Physical Therapist.
• Minimum of two years' experience in healthcare, with a preference for experience in home care.
• Valid CPR certification and driver's license with auto insurance coverage.
• Excellent communication skills and the ability to work independently.
