In this role, you will evaluate and treat patients under physician orders, utilizing therapeutic procedures to restore muscle strength, coordination, and body alignment. Your goal will be to relieve pain and improve movement and function. Collaboration with the interdisciplinary team is an essential part of this role.
Responsibilities
- Review resident information and assess its impact on treatment.
- Conduct thorough functional assessments, including home/vocational and equipment needs.
- Create and implement individualized treatment plans with achievable functional goals.
- Adhere to evaluation, treatment, and documentation guidelines.
- Instruct Assistants in treatment procedures and provide close supervision.
- Enter progress notes following each treatment session.
- Revise treatment plans as needed throughout the process.
- Document treatment course in a clear and concise discharge summary.
- Orient patients to program services and potential risks.
- Collaborate with nursing staff, patients, and families to maximize functional abilities.
- Instruct patients and caregivers on exercise programs and therapeutic procedures for continued treatment at home.
- Assist in the referral process for additional services if needed.
- Attend and participate in rehabilitation team conferences.
- Participate in other department and facility meetings.
- Access continuing education opportunities to maintain clinical competency.
- Contribute to the quality improvement process.
- Promote the programs and services of the company to the community.
- Adhere to regulatory and professional standards and guidelines.
- Provide input on the clinical performance of assistants.
- Follow facility policies, participate in quality improvement and safety programs.
- Perform other duties assigned to provide quality care to patients.
- Understand reimbursement processes and allocate resources accordingly.
- Comply with regulations and guidelines related to patient privacy and healthcare standards.
- Support the company’s commitment to Compassionate Care and project a professional image.
- Work independently and seek supervision when necessary.
- Maintain expected productivity levels for the position.

Qualifications
- Bachelor/Masters of Science degree in Physical Therapy.
- Valid license in the state of practice.
- Prefer supervision, training, or experience in a similar setting.
- Certification in specialized areas if applicable.
- Excellent communication and interpersonal skills.
- Proficiency in applying physical therapy principles, methods, materials, and equipment.
- Ability to assess patient needs and develop comprehensive care plans.
- Knowledge of accreditation standards and compliance requirements.
- Ability to perform physical tasks required for the role.
