Responsibilities:

- Create personalized treatment plans tailored to each patient's specific needs and goals.

- Regularly assess patients' progress and make necessary adjustments to treatment plans.

- Collaborate closely with a multidisciplinary team of healthcare professionals, including physicians, nurses, and occupational therapists.

- Educate patients and their families on exercises and techniques to enhance mobility and strength.

- Maintain accurate and thorough documentation of patients' progress and treatment plans, in compliance with company policies and procedures.

- Adhere to all state and federal regulations governing physical therapy practice.

Qualifications:

- Valid and up-to-date Physical Therapy license in the state.

- At least one year of clinical or home health experience.

- Extensive knowledge of physical therapy principles and practices.

- Excellent communication skills and the ability to collaborate effectively with other healthcare professionals.

- Strong ability to work independently and manage a caseload of patients.

- Must possess a valid driver's license and have reliable transportation.
