About the Role:
We are seeking a skilled and compassionate Physical Therapist (PT) to support our patients in their journey towards recovery. As a PT, you will guide and assist patients in completing their treatment plans through a variety of physical exercises and activities. You will also collaborate with physicians to determine the most effective therapy plans and document patient care services according to legal and professional requirements.
Your responsibilities will include:
- Performing Start of Care assessments
- Evaluating patient progress
- Providing continued support for patients to ensure their eligibility for home health services
- Administering maintenance therapy as needed
Qualifications:

Successful completion of an accredited Physical Therapy Program

Minimum of 1 year of experience as a physical therapist in a home health setting

Exceptional interpersonal skills and a caring demeanor
