Physical Therapist PT Home Health – Baylor Weekend
We are currently looking for a dedicated and highly skilled Physical Therapist to join our Home Health division in Buford, GA. In this position, you will be responsible for delivering exceptional, patient-centered care in the comfort of patients' homes. You will play a crucial role in creating a positive rehabilitative environment for individuals recovering from various medical conditions, surgeries, or injuries. Your main responsibilities will include assessing each patient's physical condition, developing personalized treatment plans, and implementing effective therapy interventions to enhance their independence and overall quality of life. Building strong relationships with patients, their families, and caregivers will be vital as you educate them on their conditions and guide them through the recovery process. Good communication skills, clinical expertise, and a compassionate approach are prerequisites, as you will be working with individuals from diverse backgrounds and with diverse needs. The ideal candidate will have extensive knowledge of physical therapy practices and the ability to adapt those practices to suit the home environment. - Conduct initial assessment of patients' conditions (SOC Start of Care)
- Perform evaluations to determine the most suitable treatment plans (Eval)
- Provide follow-up care as long as the patient meets eligibility requirements for home health services. Please note that Medicare does not set a limit on the number of episodes allowed for any home health benefit, including maintenance therapy.
Qualifications:
- Hold a Master's degree in physical therapy
- Have a minimum of nine months of experience as a physical therapist
- Currently licensed to practice physical therapy in the state of agency operation and adhere to accepted professional standards and practices
- Must possess and maintain a valid CPR certification while employed in a clinical role
- Must have a valid driver's license and be able to travel to all business locations
