In this role, you will assess each patient's physical condition, create personalized treatment plans, and implement therapy interventions to enhance their independence and overall quality of life. • Develop personalized treatment plans based on individual needs and goals
• Administer therapeutic exercises and training programs

• Track and document patients' progress towards rehabilitation goals
• Collaborate with other healthcare providers to ensure comprehensive patient care
• Maintain accurate patient records and documentation in compliance with regulations
Minimum Requirements:
- Master’s degree in physical therapy
- At least 1 year of experience as a physical therapist in a home health setting
- Valid and current license to practice physical therapy in the state of agency operation
Adherence to accepted professional standards and practices
- Current CPR certification
- Valid driver's license and ability to travel to various business locations
