Responsibilities:

• Perform comprehensive physical assessments and evaluate patients' medical histories.
• Create personalized treatment plans based on each patient's specific needs and rehabilitation goals.
• Administer therapeutic exercises and training programs to facilitate recovery.
• Provide education to patients and their families on managing their conditions, preventing injuries, and implementing wellness strategies.
• Monitor and document patient progress, making necessary adjustments to treatment plans to achieve rehabilitation objectives.
• Collaborate with a multidisciplinary team of healthcare professionals to ensure holistic and coordinated patient care.
• Maintain accurate and detailed patient records in compliance with healthcare regulations.

Requirements:

• Hold a master's degree in Physical Therapy.
• Have at least one year of experience as a physical therapist in a home health setting.
• Possess a current state licensure to practice as a physical therapist.
• Hold a valid CPR certification.
• Be a licensed driver with the ability to travel to different locations as required.
