We are currently looking for a dedicated and highly skilled Full-time Home Health Physical Therapist to join our team in Medford, OR. As a member of our Home Health division, you will be responsible for delivering exceptional and patient-centered care in the comfort of patients' homes. Your role will be crucial in creating a supportive environment for individuals recovering from illness, injury, or surgical procedures.

In this position, you will assess each patient's physical condition and develop personalized treatment plans to promote their independence and enhance their quality of life. You will also collaborate closely with patients, families, and caregivers to educate them about their condition and guide them through the recovery process. Effective communication skills and a compassionate approach are essential as you work with individuals from diverse backgrounds and with varying needs.

• Evaluate patients by conducting physical assessments and reviewing medical history
• Create personalized treatment plans based on individual needs and goals
• Administer therapeutic exercises and training programs
• Provide education to patients and their families regarding injury prevention and wellness
• Monitor and document patients' progress in achieving rehabilitation goals
• Collaborate with other healthcare professionals to ensure comprehensive patient care
• Maintain accurate and compliant patient records and documentation

• Master’s degree in physical therapy
• At least 1 year of experience as a physical therapist in a home health setting
• Current license to practice as a physical therapist in the state of operation
• Compliance with accepted professional standards and practices
• Valid CPR certification
• Valid driver's license with the ability to travel to various locations
