Part-time Physical Therapist (PT)

Looking for a skilled Physical Therapist to become part of our team!

Our Physical Therapists play a crucial role in helping patients with injuries and chronic health conditions recover their range of motion, manage pain, and enhance their overall quality of life. Using a diverse range of techniques, including hands-on therapy, exercises, and specialized equipment, our PTs are dedicated to providing comprehensive care to our patients.

The primary responsibilities of the Physical Therapist include evaluating and treating residents based on physician's orders. This involves using various therapeutic procedures to restore muscle strength, coordination, and body alignment, as well as alleviate pain and improve movement and function. As part of the interdisciplinary team, the Physical Therapist also contributes by providing support and information in the field of Physical Therapy.

Responsibilities

1. Assess resident information and consider its impact on the treatment process.

2. Conduct and document a thorough functional assessment of the resident, including home/vocational and equipment needs.

3. Create and implement an individualized treatment plan with achievable functional goals.

4. Adhere to evaluation, treatment, and documentation guidelines, ensuring accurate and timely records.

5. Instruct Assistants in treatment procedures, ensuring close supervision during resident interactions.

6. Maintain progress notes in the resident record after each treatment session.

7. Adjust treatment plans as necessary, ensuring continuous improvement.

8. Summarize the course of treatment in a clear and concise discharge summary format.

9. Orient residents to the program services and inform them of any potential risks during procedures.

10. Collaborate with nursing staff, residents, and families to maximize functional abilities.

11. Educate residents and caregivers about exercise programs to continue at home after treatment.

12. Assist with referral process for additional services and maintain knowledge of community resources.

13. Participate in rehabilitation team conferences and provide progress updates.

14. Attend department and facility meetings as required.

15. Pursue continuing education opportunities to maintain clinical competency within the discipline.

16. Contribute to the quality improvement process.

17. Promote the programs and services of Heritage Healthcare within the community.

18. Adhere to regulatory and professional standards and guidelines.

19. Provide feedback on the clinical performance of assistants to the Rehab Manager.

20. Follow facility policies and procedures and actively participate in quality improvement and safety programs.

21. Perform other assigned duties for quality resident care.

22. Understand reimbursement guidelines and allocate therapy resources accordingly.

23. Demonstrate knowledge of regulations and guidelines related to physical therapy practice.

24. Support the company's commitment to Compassionate Care and project a professional image.

25. Work independently, recognizing situations that require direct supervision, and willingly assist others.

26. Maintain productivity at the expected level discussed for the specific facility.

Qualifications

1. Bachelor/Masters of Science degree in Physical Therapy.

2. Current state license for practice.

3. Prefer supervision, training, or experience in a similar resident population setting.

4. Certification, if applicable, and demonstrated competency in specialized areas.

5. Excellent interpersonal and communication skills, both oral and written.

6. Demonstrated proficiency in the principles, methods, materials, and equipment used in physical therapy.

7. Ability to assess resident needs and develop comprehensive care plans.

8. Knowledge of accreditation standards and compliance requirements.

9. Ability to perform certain physical activities safely, such as standing, pushing, lifting, stooping, reaching, and balancing.
