Physical Therapist (PT) Part-time to Full-time

We are currently looking for a skilled and experienced Physical Therapist to join our team in Encinitas, CA.

As a Physical Therapist, you will play a vital role in helping patients with injuries and chronic health conditions regain their range of motion, manage their pain, and improve their quality of life. You will utilize a variety of techniques, including hands-on therapy, strengthening and stretching exercises, electrical stimulation, ultrasound, ice/heat, and more.

The main responsibility of the Physical Therapist is to evaluate and treat residents under the orders of a physician. You will use therapeutic procedures to restore muscle strength, coordination, and body alignment, relieve pain, and improve movement and function. Additionally, you will be a valuable member of our interdisciplinary team, providing support and information within the area of Physical Therapy.

Responsibilities

1. Assess available resident information and determine its impact on the assessment and treatment process.

2. Conduct a thorough functional assessment of the resident and document the findings, including assessing home/vocational and equipment needs.

3. Develop and implement an individualized treatment plan that is based on resident needs and contains achievable functional goals.

4. Ensure compliance with evaluation, treatment, and documentation guidelines, maintaining accurate department records.

5. Provide instruction to Assistants on treatment procedures and closely supervise their work with residents.

6. Record a progress note in the resident's record after each treatment session.

7. Adjust treatment plans as necessary throughout the treatment process.

8. Prepare a clear and concise discharge summary documenting the course of treatment.

9. Orient residents to program services and inform them of any potential risks during procedures.

10. Collaborate with nursing staff, residents, and families to maximize the resident's functional abilities.

11. Educate residents, family members, or other caregivers on exercise programs and therapeutic procedures to be continued at home.

12. Assist in the referral process for residents requiring additional services after discharge and maintain knowledge of community resources.

13. Attend and participate in rehabilitation team conferences to discuss resident progress, problems, and needs.

14. Take part in department/facility meetings as required.

15. Continuously engage in continuing education opportunities to maintain clinical competency.

16. Contribute to the quality improvement process.

17. Promote the programs and services of Heritage Healthcare through interactions with the community.

18. Adhere to regulatory and professional standards and guidelines set by Heritage Healthcare.

19. Provide feedback to the Rehab Manager on the clinical performance of Assistants, when requested.

20. Comply with facility policies, procedures, and participate in quality improvement and safety programs.

21. Perform any other duties as assigned to ensure quality care for residents.

22. Understand reimbursement guidelines and allocate therapy resources efficiently based on resident goals and outcomes related to discharge.

23. Demonstrate knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third-party reimbursement guidelines.

24. Support the company's commitment to Compassionate Care and maintain a professional image to foster confidence in the Rehab Program.

25. Perform duties independently, recognizing situations where direct supervision is necessary, and willingly assisting others.

26. Maintain the expected productivity level discussed and expected for your position at your facility.

Qualifications

1. Hold a Bachelor's or Master's degree in Physical Therapy.

2. Possess a current license in the state of practice.

3. Prefer supervision, training, and/or experience in a setting serving the same age/type of resident population as this facility.

4. Certification, if applicable, and demonstrated competency in specific areas of specialization.

5. Excellent oral and written communication skills, as well as strong interpersonal skills.

6. Demonstrated competency in applying physical therapy principles, methods, materials, and equipment.

7. Ability to assess resident needs and develop and implement comprehensive care plans.

8. Familiarity with accreditation standards and compliance requirements.

9. Ability to perform physical activities safely, including standing, pushing, lifting, stooping, reaching, and balancing.
