Physical Therapy Practitioner (PT) - Part-time to Full-time
We are currently seeking a highly skilled and dedicated Physical Therapist to join our team in Poway, CA!
As a Physical Therapist, you will play a crucial role in helping patients with injuries and chronic health conditions regain their range of motion, manage their pain, and improve their overall quality of life. You will utilize a variety of techniques, including hands-on therapy, exercise programs, and specialized treatments such as electrical stimulation, ultrasound, and thermotherapy.
In this role, you will be responsible for evaluating and treating residents under physician orders. You will design individualized treatment plans to restore muscle strength, coordination, and body alignment, while also providing pain relief and improving movement and function. As a member of the interdisciplinary team, you will collaborate with other healthcare professionals to ensure the best possible outcomes for our patients.
Responsibilities:
- Review resident information to assess its impact on the treatment process.
- Conduct comprehensive functional assessments, including evaluations of home/vocational and equipment needs.
- Create and implement personalized treatment plans based on resident needs and achievable functional goals.
- Adhere to evaluation, treatment, and documentation guidelines, maintaining accurate records.
- Instruct and provide close supervision to Assistants during treatment procedures.
- Document the progress of treatment in clear and concise discharge summaries.
- Revise treatment plans as required throughout the treatment process.
- Orient residents to program services and inform them of any potential risks.
- Collaborate with nursing staff, residents, and families to maximize functional abilities.
- Instruct residents and family/caregivers on home exercise programs and therapeutic procedures.
- Assist in the referral process for additional services when necessary.
- Attend and participate in team conferences and facility meetings.
- Engage in continuing education to maintain clinical competency.
- Participate in quality improvement initiatives.
- Promote the programs and services of the facility within the community.
- Adhere to regulatory and professional standards and guidelines.
- Provide feedback on the performance of Assistants, as requested.
- Follow facility policies, participate in quality improvement and safety programs.
- Perform other duties as assigned to deliver quality care.
- Understand reimbursement guidelines and allocate therapy resources efficiently.
- Comply with OBRA regulations, HIPAA, Local Coverage Determination, and third party reimbursement guidelines.
- Support the company's commitment to Compassionate Care and maintain a professional image.
- Work independently while recognizing situations that require supervision.
- Maintain expected productivity levels as discussed with the Rehab Manager.
Qualifications:
- Bachelor's or Master's degree in Physical Therapy.
- Current license to practice in the state of California.
- Experience in a similar setting serving a similar resident population (preferable).
- Certification in specialized areas of physical therapy (if applicable).
- Excellent oral and written communication skills.
- Demonstrated competency in physical therapy methods, materials, and equipment.
- Ability to assess resident needs and develop comprehensive care plans.
- Familiarity with accreditation standards and compliance requirements.
- Ability to perform physical tasks required for the role, including standing, pushing, lifting, stooping, reaching, and balancing.
