Physical Therapist (PT) Part-time to Full-time

We are currently seeking a skilled and dedicated Physical Therapist to join our team in San Pedro, CA!

As a Physical Therapist, you will play a crucial role in helping patients with injuries and chronic health conditions improve their quality of life. Using a wide range of techniques and therapies, including hands-on therapy, exercise, and electrical stimulation, you will assist patients in regaining their range of motion and managing their pain.

As a member of our interdisciplinary team, you will evaluate and treat residents under the guidance of a physician. Your primary responsibilities will include restoring muscle strength and coordination, relieving pain, and improving movement and function. You will also provide support and information within the field of Physical Therapy.

Responsibilities

1. Review resident information and assess its impact on the treatment process.

2. Conduct thorough functional assessments using standardized procedures, considering home/vocational and equipment needs.

3. Develop and implement individualized treatment plans with achievable functional goals.

4. Follow evaluation, treatment, and documentation guidelines while maintaining accurate department records.

5. Instruct Assistants in treatment procedures and provide close supervision during their interactions with residents.

6. Document treatment sessions in residents' records through progress notes.

7. Continuously revise treatment plans as necessary during the treatment process.

8. Create clear and concise discharge summaries to document the course of treatment.

9. Orient residents to program services and communicate any potential risks.

10. Collaborate with nursing staff, residents, and families to optimize functional abilities.

11. Educate residents, family members, and caregivers about post-treatment exercise programs and therapeutic procedures to be continued at home.

12. Assist in referring residents to additional services when needed.

13. Attend rehabilitation team conferences to discuss resident progress and needs.

14. Participate in department and facility meetings as required.

15. Pursue continuing education opportunities to maintain clinical competency.

16. Contribute to the quality improvement process.

17. Promote Heritage Healthcare's programs and services through community engagement.

18. Adhere to regulatory and professional standards and guidelines.

19. Provide feedback on the clinical performance of assistants to the Rehab Manager.

20. Follow facility policies and procedures and participate in quality improvement and safety programs.

21. Perform other duties assigned to deliver high-quality care to residents.

22. Allocate therapy resources efficiently based on reimbursement guidelines and resident goals and outcomes related to discharge.

23. Demonstrate knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third-party reimbursement guidelines.

24. Support the company's commitment to Compassionate Care and maintain a professional image.

25. Work independently and seek direct supervision when necessary, while fostering a helpful attitude towards others.

26. Maintain the expected productivity level for your specific facility/facilities.

Qualifications

1. Hold a Bachelor's or Master's degree in Physical Therapy.

2. Possess a current license in the state of practice.

3. Prefer supervision, training, or experience in a similar setting serving the same age/type of resident population as our facility.

4. Obtain certification, if applicable, and demonstrate competency in specialized areas.

5. Exhibit excellent oral and written communication and interpersonal skills.

6. Demonstrate competency in applying physical therapy principles, methods, materials, and equipment.

7. Capable of assessing resident needs and developing comprehensive care plans.

8. Familiarity with accreditation standards and compliance requirements.

9. Ability to safely perform standing, pushing, lifting, stooping, reaching, and balancing activities for prolonged periods as necessary.
