Physical Therapist (PT) – Part-time up to Full-time
Part-time to Full-time Physical Therapist
We are currently seeking a dedicated Physical Therapist to join our team in Lemon Grove, CA. Join us in providing exceptional care that can truly make a difference in patients' lives. As a Physical Therapist, your role will involve evaluating and treating residents based on physician orders. Through the use of various therapeutic procedures, you will help restore muscle strength, coordination, body alignment, and relieve pain to improve movement and function. You will also be an integral part of the interdisciplinary team, providing support and information within the field of Physical Therapy.
Responsibilities
1. Assess the impact of available resident information on the treatment process.
2. Perform a thorough functional assessment of each resident and document the findings.
3. Develop and implement individualized treatment plans with achievable functional goals.
4. Ensure compliance with evaluation, treatment, and documentation guidelines while maintaining accurate records.
5. Teach treatment procedures to Assistants and supervise their work with residents.
6. Enter progress notes in the resident record after each treatment session.
7. Revise treatment plans as needed throughout the process and provide clear discharge summaries.
8. Orient residents to program services and inform them of any potential risks during procedures.
9. Collaborate with nursing staff, residents, and families to maximize functional abilities.
10. Instruct residents and caregivers on exercise programs and therapeutic procedures for continued use at home.
11. Assist in the referral process for additional services and stay updated on community resources.
12. Participate in rehabilitation team conferences and other department meetings.
13. Attend continuing education opportunities to maintain clinical competency.
14. Contribute to the quality improvement process and promote the programs and services of Heritage Healthcare.
15. Follow regulatory and professional standards and guidelines set by Heritage Healthcare.
16. Provide feedback to the Rehab Manager on the performance of assistants if requested.
17. Adhere to facility policies and participate in quality improvement and safety programs.
18. Perform other duties as assigned to ensure quality care for residents.
19. Allocate therapy resources efficiently based on resident goals and reimbursement guidelines.
20. Demonstrate knowledge of regulations and guidelines related to therapy.
21. Project a professional image and maintain productivity levels as expected.

Qualifications
1. Bachelor/Masters of Science degree in Physical Therapy.
2. Current license in the state of practice.
3. Prefer experience in a setting serving a similar resident population.
4. Certification and demonstrated competency in specialized areas, if applicable.
5. Excellent communication and interpersonal skills, both written and oral.
6. Proficiency in applying principles, methods, materials, and equipment used in physical therapy.
7. Ability to assess resident needs and develop comprehensive care plans.
8. Knowledge of accreditation standards and compliance requirements.
9. Ability to perform various physical activities safely and for prolonged periods as required for the job.
