Physical Therapist (PT) Part-time
We are currently seeking a passionate and dedicated Physical Therapist to join our dynamic team in Chula Vista, CA.
As a Physical Therapist, you will play a crucial role in helping patients recover from injuries and manage chronic health conditions. Your main responsibilities will involve evaluating patients, developing personalized treatment plans, and implementing various therapeutic procedures to restore muscle strength, improve coordination, relieve pain, and enhance movement and function. You will work closely with a multidisciplinary team to ensure the best possible outcomes for each patient.
Responsibilities
- Evaluating available resident information and considering its impact on the assessment and treatment process
- Conducting comprehensive functional assessments, including home/vocational and equipment needs
- Creating individualized treatment plans based on resident needs and achievable functional goals
- Ensuring compliance with evaluation, treatment, and documentation guidelines, while maintaining accurate department records
- Instructing and supervising Assistants in treatment procedures
- Recording progress notes following each treatment session
- Revising treatment plans as necessary throughout the treatment process
- Documenting treatment courses in clear and concise discharge summaries
- Orienting residents to program services and procedures, while informing them of any potential risks
- Collaborating with nursing staff, residents, and families to maximize functional abilities
- Instructing residents and their families/caregivers on exercise programs and therapeutic procedures for continued treatment at home
- Assisting in the referral process for additional services when required
- Participating in rehabilitation team conferences to discuss resident progress, problems, and needs
- Attending departmental meetings and other facility meetings as required
- Continuously seeking and participating in relevant continuing education opportunities
- Contributing to the quality improvement process
- Promoting the programs and services of Heritage Healthcare through community interactions
- Adhering to regulatory and professional standards and guidelines
- Providing feedback on the clinical performance of Assistants to the Rehab Manager
- Following facility policies, procedures, and participating in quality improvement and safety programs
- Performing additional duties as assigned and required to provide quality care to residents
- Understanding reimbursement procedures from various payers and allocating therapy resources efficiently
- Demonstrating knowledge of relevant regulations and reimbursement guidelines
- Supporting the company's commitment to compassionate care and maintaining a professional image
- Working independently and recognizing situations where direct supervision is needed
- Maintaining expected productivity levels
Qualifications
- Bachelor's or Master's degree in Physical Therapy
- Current license in the state of practice
- Prefer supervision, training, and/or experience in a similar facility
- Certification and demonstrated competency in specialized areas, if applicable
- Excellent oral and written communication and interpersonal skills
- Demonstrated competency in physical therapy techniques and equipment
- Ability to assess resident needs and develop comprehensive care plans
- Knowledge of accreditation standards and compliance requirements
- Ability to perform physical activities required for the role
