We are seeking a skilled Physical Therapist to join our team in West Los Angeles, CA.

The role of the Physical Therapist is to evaluate and treat patients based on physician orders. Utilizing various therapeutic procedures, the Physical Therapist aims to restore muscle strength, coordination, and body alignment, alleviate pain, and improve movement and function. Additionally, the Physical Therapist collaborates with an interdisciplinary team, providing support and information within the field of Physical Therapy.

Responsibilities

1. Assess available resident information and evaluate its impact on the assessment and treatment process.

2. Conduct a thorough functional assessment of the resident and document the findings, including home/vocational and equipment needs.

3. Develop and execute an individualized treatment plan based on the resident's specific needs and achievable functional goals.

4. Adhere to evaluation, treatment, and documentation guidelines, ensuring timely and accurate department records.

5. Provide instruction to Assistants on treatment procedures and closely supervise their work with residents.

6. Enter a progress note in the resident record after each treatment session.

7. Adjust treatment plans as necessary throughout the treatment process.

8. Document the course of treatment in a clear and concise discharge summary.

9. Orient residents to program services and inform them of any potential risks associated with procedures.

10. Collaborate with nursing staff, residents, and families to maximize functional abilities.

11. Educate residents, family members, or other caregivers on exercise programs and therapeutic procedures they can continue at home.

12. Assist in the referral process for residents requiring additional services after discharge and stay informed about community resources.

13. Attend and actively participate in rehabilitation team conferences to discuss resident progress, problems, and needs.

14. Attend other department/facility meetings, as required.

15. Pursue continuing education opportunities to maintain clinical competency.

16. Contribute to the quality improvement process.

17. Promote the programs and services of our company through interactions with the community.

18. Adhere to regulatory and professional standards and guidelines.

19. Provide feedback to the Rehab Manager on the clinical performance of Assistants, when appropriate.

20. Follow facility policies and procedures and participate in quality improvement and safety programs.

21. Perform additional duties as assigned to ensure quality care for residents.

22. Understand reimbursement by various payers and allocate therapy resources efficiently based on resident goals and outcomes related to discharge destination.

23. Demonstrate knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third-party reimbursement guidelines.

24. Support our commitment to Compassionate Care and project a professional image that instills confidence in the Rehab Program.

25. Work independently when appropriate but also recognize when direct supervision is necessary and assist others willingly.

26. Maintain the expected productivity level discussed for your position at your particular facility/facilities.

Qualifications

1. Hold a Bachelor/Masters of Science degree in Physical Therapy.

2. Possess a current license in the state of practice.

3. Prefer supervision, training, or experience in a setting serving a similar resident population as our facility.

4. Certification, if applicable, and demonstrated competency in specialized areas.

5. Showcase excellent oral and written communication as well as interpersonal skills.

6. Demonstrate competency in applying physical therapy principles, methods, materials, and equipment.

7. Ability to assess resident needs and develop and implement comprehensive care plans.

8. Knowledge of accreditation standards and compliance requirements.

9. Capability to perform the following activities safely and for extended periods of time, if necessary:

• Standing: Maintain an upright position on your feet

• Pushing: Apply pressure to move objects forward or away from you - push or pull force up to 35 lbs.

• Lifting: Raise objects from lower to higher positions or move objects laterally

• Stooping: Bend forward from the waist, hips, and knees

• Reaching: Extend arms and hands away from the body

• Balancing: Maintain equilibrium to prevent falling on various surfaces
