We are currently seeking a highly skilled and dedicated Physical Therapy Specialist to join our team in Carson, CA. As a Physical Therapy Specialist, you will be responsible for assessing and treating patients under physician orders, using various therapeutic procedures to restore muscle strength, coordination, and body alignment. Your role will also involve relieving pain and improving movement and function. You will collaborate with a interdisciplinary team, providing support and information in the area of Physical Therapy.
Responsibilities
1. Evaluate resident information and consider its impact on the assessment and treatment process.
2. Conduct a comprehensive functional assessment of each resident, using standardized procedures. Assess home/vocational needs and equipment requirements.
3. Develop and implement personalized treatment plans that address the specific needs of each resident, focusing on achievable functional goals.
4. Adhere to evaluation, treatment, and documentation guidelines, ensuring accurate and timely department records.
5. Instruct Assistants on treatment procedures and provide close supervision while they work with residents.
6. Document the progress of each treatment session in the resident record.
7. Adjust treatment plans as necessary throughout the treatment process.
8. Summarize the course of treatment in a clear and concise discharge summary.
9. Orient residents to program services and inform them of any potential risks during procedures.
10. Collaborate with nursing staff, residents, and families to maximize functional abilities.
11. Educate residents, families, and caregivers on exercise programs and therapeutic procedures for continued use at home.
12. Assist in the referral process for residents requiring additional services after discharge.
13. Participate in rehabilitation team conferences to discuss resident progress, problems, and needs.
14. Attend department and facility meetings as required.
15. Pursue continuing education opportunities to maintain clinical competency.
16. Contribute to the quality improvement process.
17. Promote the programs and services of Heritage Healthcare through interactions with the community.
18. Adhere to regulatory and professional standards and guidelines.
19. Provide feedback to the Rehab Manager on the performance of assistants, as requested.
20. Comply with facility policies, participate in quality improvement, and safety programs.
21. Perform additional duties as assigned to ensure quality care for residents.
22. Utilize therapy resources efficiently based on resident goals and discharge outcomes related to reimbursement by various payers.
23. Demonstrate knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third party reimbursement guidelines.
24. Support the company’s commitment to Compassionate Care by projecting a professional image and fostering confidence in the Rehab Program.
25. Work independently but seek supervision when needed, and willingly assist others.
26. Maintain productivity levels discussed and expected for your position and facility.

Qualifications
1. Bachelor's or Master's degree in Physical Therapy.
2. Valid state license for practice.
3. Prefer supervision, training, or experience in a similar setting serving the same population as this facility.
4. Certification in specialized areas, if applicable, and demonstrated competency.
5. Excellent communication and interpersonal skills.
6. Competency in applying physical therapy principles, methods, materials, and equipment.
7. Ability to assess resident needs and develop comprehensive care plans.
8. Knowledge of accreditation standards and compliance requirements.
9. Ability to perform physical tasks safely and for extended periods.
• Standing: Maintaining an upright position on feet, including prolonged periods.
• Pushing: Applying pressure to move objects forward or away, up to 35 lbs.
• Lifting: Raising objects from lower to higher positions, independently or with assistance.
• Stooping: Bending forward and down from waist, hips, and knees for prolonged periods.
• Reaching: Extending hands and arms in any direction away from the body.
• Balancing: Maintaining equilibrium when walking, standing, or crouching on narrow or slippery surfaces.
