Physical Therapist (PT) Per Diem
We are currently looking for an enthusiastic and dedicated PRN Physical Therapist to join our team in Irvine, CA. This position is perfect for someone who is passionate about providing personalized care to clients in need of physical therapy. As a Physical Therapist, you will play a vital role in helping clients restore muscle strength, coordination, and body alignment, alleviate pain, and improve overall movement and function. About the Role:
In this role, you will evaluate and treat residents, as directed by a physician, using a variety of therapeutic techniques to address muscle strength, coordination, body alignment, pain relief, and movement improvement. You will work as part of a collaborative interdisciplinary team, providing support and information in the area of Physical Therapy.
Responsibilities:
- Review resident information and assess its impact on the treatment process.
- Conduct a comprehensive functional assessment of residents, including their home/vocational and equipment needs.
- Create and implement personalized treatment plans that align with residents' goals and needs.
- Adhere to evaluation, treatment, and documentation guidelines, ensuring accurate and timely records.
- Instruct and supervise Assistants in treatment procedures.
- Provide progress notes after each treatment session.
- Revise treatment plans as necessary throughout the process.
- Compose clear and concise discharge summaries to document the course of treatment.
- Orient residents to program services and inform them of any potential risks.
- Collaborate with nursing staff, residents, and families to maximize functional abilities.
- Instruct residents and their caregivers on exercise programs and therapeutic procedures for continued treatment at home.
- Assist in the referral process for additional services as needed.
- Participate in rehabilitation team conferences to discuss resident progress, problems, or needs.
- Attend department/facility meetings and seek continuing education opportunities to maintain clinical competency.
- Support and promote the programs and services of our organization to the community.
- Act in compliance with regulatory and professional standards and guidelines.
- Provide feedback on the performance of Assistants, as requested.
- Follow facility policies, participate in quality improvement, and adhere to safety programs.
- Perform any additional duties assigned to provide quality care to residents.
- Demonstrate knowledge of reimbursement guidelines and allocate therapy resources accordingly.
- Stay updated on OBRA regulations, HIPAA, Local Coverage Determination, and third-party reimbursement guidelines.
- Represent our organization with professionalism to instill confidence in the Rehab Program.
- Work independently, seeking supervision when necessary, and be willing to assist others.
- Maintain expected productivity levels as discussed and expected for the specific facility/facilities.

Qualifications:
- Hold a Bachelor/Master of Science degree in Physical Therapy.
- Have a valid license to practice Physical Therapy in the state of California.
- Prefer supervision, training, or experience in a similar setting as our facility.
- Show certification, if applicable, and proficiency in specialized areas.
- Possess excellent verbal and written communication skills.
- Demonstrate competence in applying principles, methods, materials, and equipment used in physical therapy.
- Be able to assess residents' needs and develop comprehensive care plans.
- Show familiarity with accreditation standards and compliance requirements.
- Be capable of performing physical activities necessary for the role, such as standing, pushing, lifting, stooping, reaching, and balancing.
