Physical Therapist PT
We are in search of a committed and skilled Rehabilitation Specialist for our Home Health division to deliver top-notch, patient-focused care in the home environment. This position is vital in creating a positive rehabilitative atmosphere for individuals recovering from sickness, injuries, or surgeries. As a Rehabilitation Specialist in home health, your responsibilities will include assessing each patient's physical condition, devising personalized treatment plans, and implementing therapy interventions aimed at maximizing independence and quality of life. You will closely collaborate with patients, families, and caregivers to educate them about their conditions and the necessary steps for recovery. This role demands effective communication skills, clinical expertise, and a compassionate approach as you work with individuals from diverse backgrounds and with varying needs. • Create individualized treatment plans based on specific requirements and goals

• Maintain accurate patient records and adhere to regulatory standards
Qualifications:
- Master’s degree in physical therapy
- Minimum of 1 year of experience as a physical therapist in home health
- Valid license to practice as a physical therapist in the state of agency operation, and compliant with accepted professional standards and practices
- Must possess and maintain a current CPR certification while employed in a clinical role
- Must have a valid driver's license and the ability to travel to all business locations
