Position Summary:

  • This role primarily involves working in an outpatient setting, with a one-to-one ratio of patients to therapists.
  • Responsibilities include providing a complete range of physical therapy services, such as assessing patients, creating treatment plans, and implementing therapeutic interventions. This position emphasizes interdisciplinary collaboration and adherence to professional qualifications, ethical standards, and practices. The Physical Therapist will also contribute to program development, problem-solving, quality improvement, and productivity enhancement within an interdisciplinary environment.

Education, License & Certifications:

  • Minimum requirement: a graduate of an accredited four-year Bachelor's degree program (or equivalent from a foreign institution).
  • Must hold a current license in the state of practice.
  • Mandatory current BLS (Basic Life Support) Certification.


  • Education and experience in assisting patients with disabilities or injuries across all age groups throughout their rehabilitation process.

Essential Functions:

1. Maintain a professional image and demonstrate effective communication skills with patients, families, coworkers, and other stakeholders.

2. Conduct thorough initial assessments, utilizing appropriate assessment tools and adaptive equipment/materials suitable for the patient's age and developmental level. Document assessment findings accurately in the medical record in compliance with policies and procedures.

3. Develop customized treatment plans with both short and long-term objectives that are measurable and relate specifically to the patient's functional deficits. Choose appropriate interventions based on assessment results, patient age, cognitive abilities, and developmental stage, which may involve administering prescribed medication.

4. Continuously evaluate the patient's response to treatment, regularly revising the treatment plan based on progress or the need for adjustment. Provide patients and their families with updates on their current status, ongoing program planning, and discharge planning throughout the rehabilitation program.

5. Document the patient's progress accurately, justifying the need for skilled intervention or recommending further clinical actions if progress is lacking. Complete daily progress notes in adherence to policies and procedures. Outpatient progress notes should be completed at least every 10 visits or as required by state practice regulations. Ensure proper recording of charges related to provided treatment. Prepare discharge summaries according to established policies and procedures.

6. Provide patient and family education on tasks and resources that promote continued functional independence. Engage family members in the treatment, educational, and discharge planning processes to facilitate a safe transition home and ensure ongoing care needs are met. Measure the effectiveness of education through peer input, verbal feedback from families, patient satisfaction surveys, and quality metrics. Adapt educational methods to meet the specific needs of patients and their families. Collaborate with the multidisciplinary team, if necessary, for comprehensive discharge planning and determining the appropriate next level of care.

7. Actively participate in continuing education and professional development activities. Attend and present at departmental in-service sessions as per the schedule. Attend external professional growth activities and share valuable information with the team.

8. Adhere to relevant federal and state guidelines, professional organization standards, facility policies, and professional codes of conduct. Familiarize oneself with the contents of the professional code, employee handbook, unit Policy & Procedure, and organizational policies, and act accordingly.

9. Achieve established productivity standards, ensuring fiscally responsible practices.

Other Duties:

  • Perform all required tasks and operate equipment within the Physical Therapy Department, as outlined in the Competency Checklist.
