Position Summary:

  • Responsible for delivering a comprehensive range of physical therapy services, including evaluation, treatment planning, and therapeutic interventions within a collaborative environment that aligns with the role's qualifications, professional standards, and ethical principles. The PT will also contribute to program development, quality improvement, problem-solving, and productivity enhancement in an interdisciplinary setting.

Education, License & Certifications:

  • The successful candidate must possess a minimum of a four-year Bachelor's degree from an accredited program (or its foreign equivalent).
  • Current licensure in the state of practice is mandatory.
  • Current BLS Certification is a requirement.


  • The ideal candidate should have education and experience working with patients to aid in the rehabilitation of individuals with disabilities or injuries across all age groups.

Essential Functions:

1. Establish and maintain professional and effective communication with patients, families, clients, colleagues, and other relevant stakeholders.

2. Perform initial assessments, utilizing appropriate assessment tools and adaptive equipment/materials suitable for the patient's age and developmental stage. Document assessments accurately in the medical records according to policy. Ensure all data is recorded in the appropriate sections.

3. Develop customized treatment plans that outline short and long-term goals, objectively and measurably targeting specific functional deficits requiring ongoing treatment. Select interventions based on assessment results, considering the patient's age, cognitive abilities, and developmental level. Administer medications as prescribed, if necessary.

4. Evaluate the patient's response to the treatment, making revisions to the treatment plan as required. Update the patient and their family on progress, program planning, and discharge plans throughout the rehabilitation program.

5. Accurately document the patient's progress to justify the need for skilled intervention, monitor lack of progress, and provide further clinical recommendations to referring providers. Record interventions and their effects daily following set policies and procedures. Complete daily progress notes and discharge summaries according to policy and procedure. Ensure appropriate charging for provided treatments.

6. Educate patients and their families on tasks and available resources that promote ongoing functional independence. Involve family members in the treatment, education, and discharge planning process to ensure a safe transition home and continued quality care. Measure the effectiveness of education through peer input, verbal feedback from families, patient satisfaction surveys, and quality metrics. Adapt educational methods to meet the specific needs of each patient and their family. Collaborate with the multidisciplinary team on discharge planning and determining the appropriate next level of care, when necessary.

7. Engage in continuing education and professional development activities. Attend or present at departmental in-service sessions as scheduled. Attend external professional growth activities as required and share pertinent information with the team.

8. Adhere to the policies and practices outlined in federal and state guidelines, professional organizations, facility regulations, and entity-specific policies. Demonstrate familiarity with and compliance with relevant professional codes and ethics, employee handbooks, unit policies and procedures, and organizational policies.

9. Meet established organizational productivity standards in a financially responsible manner.

Other Duties:

  • Perform all tasks and operate all equipment within the Physical Therapy Department as listed on the Competency Checklist.
