Summary:

  1. Conduct thorough assessments of residents upon admission and formulate and implement individualized therapy plans to effectively manage their conditions, prevent further impairment, and promote long-term wellness.
  2. Emphasize the improvement of movement, pain reduction, and disability prevention through exercise therapy, manual techniques, and other cutting-edge methodologies.

Duties Include:

  • Evaluate the condition of each resident and develop an appropriate plan for physical rehabilitation.
  • Assist residents in enhancing their mobility.
  • Alleviate pain through exercises, hands-on therapy, stretching, and the use of physical therapy devices.
  • Provide instruction to residents on the proper use of physical therapy equipment.
  • Support and monitor residents in performing tasks and assess their progress.
  • Inform residents about the anticipated stages of recovery and provide guidance for each step.
  • Maintain confidentiality to ensure resident trust and safeguard facility operations.
  • Supervise the activities of physical therapy assistants when necessary.
  • Ensure a safe and clean work environment.
  • Adhere to infection-control policies and protocols to protect residents and staff.
  • Comply with all federal, state, and local legal and professional requirements.
  • Collaborate with interdisciplinary teams to provide comprehensive resident care.

QUALIFICATIONS:

  • Completion of an accredited Physical Therapy Program
  • Valid PT License in the state of New York
  • Prior experience in skilled nursing or long-term care settings is preferred.
