PRIMARY CARE PHYSICIAN

Location: Palm Desert, CA

Salary: 250-300K Base Salary + Annual Incentive Opportunity + Paid Relocation + Full Benefits + EQUITY! +SIGNING BONUS

We are seeking a skilled Primary Care Physician to join our team in Palm Desert, CA. As a Primary Care Physician, you will be responsible for providing comprehensive assessments, developing care plans, and overseeing the care of participants in nursing homes and hospitals. You will also provide guidance and support to Nurse Practitioners and other team members.

Essential Job Duties:

• Conduct comprehensive assessments and provide primary care to participants
• Collaborate with community-based physicians and interdisciplinary teams in discharge planning
• Participate in interdisciplinary team meetings and coordinate 24-hour care delivery
• Maintain regular visits to participants in nursing homes
• Participate in rotating night and weekend call

How you will make an impact:

· Interview, examine, and treat clinical patients

· Educate patients and families on wellness, prevention, and early detection

· Provide referrals based on examination and patient needs

· Participate in quality improvement, management, continuing education, and patient care programs

· Assist in resolving complaints, requests, and inquiries from patients

· Maintain patient confidentiality according to federal guidelines and regulations

· Complete daily documentation of medical records (EMR)

· Cover night calls as needed

Job Requirements:

• Completion of an accredited residency program
• Doctorate Degree
• Board Certification in Internal Medicine, Family Practice, or Geriatrics (preferred)
• Active California Physician License without restrictions
• Active state CDS Certification and DEA
• Enrollment or eligibility for enrollment to provide services to Medicare and Medi-Cal participants
