PRIMARY CARE PHYSICIAN

We are offering a competitive salary of 250-300K, along with an annual incentive opportunity, paid relocation, full benefits, equity, and a signing bonus. This is a full-time clinical onsite role located in Coachella, CA.

The Primary Care Physician will be responsible for conducting comprehensive assessments and working with a team to create personalized care plans for each patient. Under the guidance of the Medical Director, the Primary Care Physician will oversee all aspects of care for patients in nursing homes and hospitals, including admissions, discharges, and coordination with medical specialists. Additionally, the Primary Care Physician will provide direction and support to Nurse Practitioners and other team members.

Key Responsibilities:

• Conduct assessments, create care plans, and provide ongoing primary care to patients
• Collaborate with community-based physicians and the interdisciplinary team (IDT) for discharge planning
• Participate actively in IDT meetings, communicating changes in patient condition and coordinating care
• Maintain ongoing care for patients in nursing homes, making regular visits as needed
• Participate in night and weekend call rotations

How You Will Make an Impact:

· Interview, examine, and treat patients to provide comprehensive medical care

· Educate patients and families on wellness, prevention, and early detection

· Make referrals based on examination and patient needs

· Contribute to quality improvement, management, and continuing education initiatives

· Assist in addressing patient complaints, requests, and inquiries

· Ensure confidentiality of patient information in accordance with regulations

· Complete daily documentation of medical records (EMR)

· Cover night calls as required

Job Requirements:

• Successful completion of an Accreditation Council for Graduate Medical Education (ACGME) accredited residency program
• Doctorate Degree
• Board Certification in Internal Medicine, Family Practice, or Geriatrics (preferred)
• Active California Physician License without Restrictions
• Active state Certification of Drug and Alcohol Services (CDS) and DEA registration
• Enrollment or eligibility for enrollment to provide services to Medicare and Medi-Cal patients
