Per Diem Licensed Medical Social Worker - Hospice

Special Instructions: You must have experience in the hospice and/or home health industry to be considered for this position, without exception.

Summary of the Role:
The main responsibility is to provide coordinated care to patients of all age groups in their homes. This includes conducting psycho-social assessments, analysis, counseling, and making referrals to address the needs of the patient and their family.

Responsibilities:

• Evaluate hospice patients to determine their psychosocial, grief, financial, environmental, and community needs, as evidenced by documentation in the Plan of Care (POC), clinical records, team meeting minutes, and community resource referrals
• Provide individual counseling to patients and their families, offer crisis intervention, assist with advance directives, funeral planning issues, and facilitate the transfer of responsibilities related to legal and fiscal matters
• Coordinate community resources based on the specific needs of the patient and their family

Qualifications:
Education: Bachelor's or Master's Degree in Social Work from a program accredited by the state.
Licensure: Certification from the state.
Experience: Minimum of one (1) year of social worker experience in a healthcare agency.
