PSYCHIATRIC MENTAL HEALTH NURSE PRACTITIONER / HYBRID REMOTE

$160,000.00 - $185,000.00 + Bonus + Equity + Signing Bonus

As directed by the Medical Director, the Psychiatric Mental Health Nurse Practitioner collaborates with PACE Physicians and the IDT team to provide assessment, planning, primary care, and continuing medical education to participants.

This program enables seniors to continue living in their homes and communities.

Key Responsibilities:

• Collaborate with PACE Physicians and Community-Based PACE Physicians to provide psychiatric consultation, evaluation, counseling, and treatment for participants
• Perform comprehensive psychiatric assessments on participants, either in person or via telehealth, in collaboration with a physician, and develop individualized care plans with the IDT team
• Work with physician(s) and IDT to plan discharge from acute or long-term care settings
• Refer participants to a psychiatrist when necessary, in consultation with the PACE Physician and/or Community-Based PACE Physician
• Oversee the care of participants in nursing homes, making regular visits as per participant needs, in collaboration with the PACE Physician and/or Community-Based PACE Physician
• Act as a consultant to the IDT, sharing participant changes, collaborating on care planning arrangements, and coordinating 24-hour care delivery

Job Requirements:

• Hold a Master's degree in nursing with a Psychiatric/Mental Health emphasis from an accredited college or university
• Be certified as a Psychiatric Mental Health Nurse Practitioner
• Hold an active Psychiatric Mental Health Nurse Practitioner State License without restrictions
• Possess State CDS Certification and DEA License
• Have an NPI license
• Hold a valid State Driver's License with a clean DMV record
• Provide a copy of recent Vehicle Insurance
• Have a minimum of two (2) years of Psychiatric Mental Health experience working with the frail or elderly

Must Have

• Certificated as a Psychiatric Mental Health Nurse Practitioner
• Licensed in California
• Active DEA
