Role and Responsibilities -
As the Regional Director, you will be responsible for planning, organizing, directing, coordinating, and evaluating patient
services at the regional level. You'll work closely with clinical management to ensure high-quality outcomes, patient and staff
satisfaction, and achievement of revenue goals. While not mandatory, the Regional Director may also serve as the
Director of Patient Services.

Essential Functions
· Take part in the development, implementation, evaluation, review, and revision of policies and
procedures that align with current professional standards of practice.
· Develop relevant goals, objectives, and care standards for patient services and personnel functions.
· Ensure coordination of all patient services by monitoring treatment plans, organizing case conferences,
periodically evaluating patients, and determining staffing patterns and frequency.
· Coordinate the quarterly clinical record review program.
· Maintain effective communication and coordination with physicians, payers, patients/families,
outpatient services, inpatient coordinators, and other relevant parties regarding patient care and status.
· Ensure that decisions regarding patient referral, admission, and discharge prioritize patient needs and
safety.
· Facilitate comprehensive interdisciplinary assessments that address the medical, social, mental health,
and environmental needs of each patient.
· Provide adequate supervision of personnel delivering patient care:
o Monitor compliance with patient care plans
o Review clinical documentation
o Conduct in-home personnel supervision
· Create personnel schedules to ensure proper coverage within authorized staffing levels.
· Recruit qualified personnel and implement strategies to promote staff retention, in collaboration with
Clinical Managers and HR Department.
· Ensure staff members maintain up-to-date credentials, adhere to job responsibilities and productivity
requirements, and receive timely and thorough performance reviews.
· Participate in the agency’s information management and performance improvement programs.
· Serve as a member of the Professional Advisory Council and periodically review policies related to
patient care with the council.
· Participate in local organizations and activities related to health professions and community health
services.
· Oversee ongoing agency functions, including but not limited to:
- Ensuring agency performance in financial, quality, and regulatory aspects
- Managing operations within the allocated budget
- Staying up-to-date with Medicare, Medicaid, and third-party payer requirements for home care
reimbursement

- RN with BSN credentials required.
- The ideal candidate is a registered professional nurse, physical or occupational therapist, or medical social worker, with a bachelor’s degree (master’s preferred), and at least two (2) years of experience in both home health care and a clinical management position.
- Valid professional licensure in the state of New York.
- Minimum of two years’ experience in home health care.
- Prior experience with Medicare home health and management is preferred.
- Exceptional organization, attention to detail, and effective oral and written communication skills.
- Demonstrated understanding of delivering home health care and the ability to effectively problem-solve.
- Ability to prioritize and handle multiple critical tasks simultaneously.
- Adaptable to change and interruptions.
- Strong management skills.
- Must have a valid driver’s license, automobile liability insurance, and reliable transportation capable of driving in all weather conditions.
