Hybrid Work Environment: Work remotely 1-2 days per week and in the office 2-3 days per week

The Nutritional Health Specialist is responsible for evaluating, educating, and managing patients with chronic conditions who are at risk for disease progression, complications, cognitive and functional decline. Working alongside physicians and interdisciplinary teams, the Nutritional Health Specialist develops and implements personalized care plans to enhance patients' understanding of their condition, promote healthy lifestyles, and enhance clinical outcomes. This role also involves creating and delivering nutritional programs for the provider organization, catering to both adults and children.

Key Responsibilities

  • Conduct thorough nutritional assessments, education, and management for patients referred by physicians or the multidisciplinary care team, either in-home, at the primary care practice, or remotely via telephone
  • Provide guidance to clients on modified diets to promote health and manage chronic conditions
  • Coordinate, implement, and evaluate nutritional education programs based on national standards and guidelines
  • Evaluate patients' needs for services (based on HEDIS metrics) and assist in closing gaps in required healthcare services (preventative and condition-specific)
  • Create and deliver educational programs and teaching tools tailored to various ethnic and cultural groups to enhance nutrition understanding
  • Offer outpatient and community-based education and counseling to individuals and families, either one-on-one or in groups, as per established educational plans
  • Prepare nutrition-related presentations and deliver them to diverse audiences
  • Maintain complete and timely documentation and billing for all care management encounters

Qualifications

  • Bachelor's degree in nutrition or a related field, with coursework approved by the American Dietetic Association's Commission on Accreditation for Dietetics Education
  • Successful completion of the national examination administered by the Commission on Dietetic Registration
  • Knowledge of nutrition guidelines for managing chronic diseases
  • Familiarity with medical terminology
  • Understanding of HEDIS or NCQA quality measures
  • Valid and unrestricted driver's license, with the ability to travel to multiple locations

Skills and Abilities

  • Ability to work independently and establish productive relationships with healthcare providers from different disciplines
  • Knowledge of principles and case management guidelines related to the assigned program area
  • Awareness of the unique cultural, linguistic, health literacy, and age-specific care needs of diverse patients and caregivers
  • Advanced skills in patient interviewing and assessment
  • Exceptional interpersonal and communication skills
  • Strong problem-solving, analytical, and decision-making abilities
  • Up-to-date knowledge of standards of care and clinical guidelines for patients with chronic illnesses
  • Effective collaboration within a multidisciplinary team
  • Ability to work autonomously, maintain a workload at multiple sites, prioritize tasks, and ensure productivity through proactive case finding and referrals
  • Working knowledge of insurance and regulatory guidelines for practice
  • Understanding of HEDIS or NCQA quality measures
  • Proficiency in computer applications relevant to the role, including Microsoft Office Suite (Word and Excel), electronic medical records, and patient registries

Job Type: Full-time

Salary: $75,000.00 - $95,000.00 per year
