Registered Nurse, RN – Full-Time (Home Health)

Responsibilities
- Deliver direct patient care to individuals of various ages and health conditions.
- Conduct comprehensive assessments and accurately document patient medical histories.
- Create and implement personalized care plans based on individual patient needs and conditions.
- Administer prescribed medications and treatments under the guidance of physicians.
- Monitor and record vital signs, symptoms, and changes in patient conditions.
- Collaborate with interdisciplinary teams to ensure comprehensive care and achieve optimal patient outcomes.
- Educate patients and families on health management, disease prevention, and available treatment options.
- Maintain precise and up-to-date patient records and documentation.
Requirements
- Current California Registered Nurse (RN) license without any disciplinary actions.
- Valid California driver's license, vehicle insurance, and reliable transportation are required.
- Current CPR certification is mandatory.
- Minimum of one year recent/active experience in an acute care setting.
- Willingness to drive to and provide care at patients' places of residence.
- Demonstrated excellent clinical skills in patient care provision.
- Proficient in documenting care according to company standards and state and federal guidelines.
- Legible writing skills are essential.
- Ability to meet productivity levels set by the agency, which may vary based on field responsibilities and employment status.
- Adherence to quality standards regarding documentation.
- Successful completion of the Home Health Nurse Exam and Skills Self Evaluation. Must achieve a score of 80% or higher on the exam.
- Effective communication skills are a must.
- Capability to chart at the bedside.
- Aptitude for working well with other team members.
- No health restrictions that hinder the provision of care to patients.
- Ability to stoop, bend, and lift up to 50 pounds, with no physical work restrictions.
