Duties

• Perform comprehensive assessments of patients to determine their healthcare requirements.
• Collaborate with patients and their families to develop customized care plans.
• Administer prescribed medications and treatments as directed by physicians.
• Monitor and record patient progress and response to treatment.
• Provide education to patients and caregivers on disease management and maintaining good health.
• Coordinate with interdisciplinary teams to deliver comprehensive patient care.
• Timely response to patient inquiries and provide necessary support.

Qualifications

• Active and unrestricted nursing license in the State of California.
• Minimum 1-year professional nursing experience in Home Health or Acute Care settings.
• Valid Driver's License with a clean driving record
