Position Overview
As an RN Care Manager, you will be part of our Care Management Team, ensuring seamless care coordination for patients. Case Management is a collaborative discipline involving patients, caregivers, nurses, social workers, physicians, payers, support staff, and community resources. This role focuses on effective transitional care management and utilization management, while recognizing the social determinants of health that impact patient care.

Required Qualifications
✔ Bachelor’s Degree in Nursing (or equivalent education/experience)
✔ California Registered Nurse License (upon hire)
✔ 2+ years of experience in a healthcare-related setting (Acute, Ambulatory, Post-Acute, etc.)
✔ 2+ years of experience in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Managers. (TIP candidates must have experience in the same nursing unit as the available CM position.)
Additional Notes:
- RNs without a BSN must obtain one within three years of hire (exceptions considered on a case-by-case basis).
- RNs employed prior to August 2021 are encouraged to obtain a BSN but are not required to do so.
Preferred Qualifications
➤ Master’s Degree in Nursing or a Healthcare-related field
➤ Case Management Certification (ACM or CCM)
