RN Clinical Manager Home Health

Home Health Clinical Manager (RN)

Schedule: Full Time | 8:00 AM – 5:00 PM

We are seeking an experienced and motivated Clinical Manager to oversee clinical operations in a home health setting. This is a key leadership role responsible for ensuring the delivery of high-quality, compliant patient care through effective oversight and coordination. While this role does not include direct supervision of a team of clinicians, it plays a critical part in guiding and supporting clinical workflows, quality initiatives, and operational effectiveness.


Key Responsibilities

Clinical Leadership & Compliance

  • Oversee daily clinical and office operations; remain accessible to staff and patients during business hours

  • Ensure all services adhere to professional standards, agency policies, and state/federal regulations

  • Provide backup support for key roles, including PCM and CSS; act as the leader in the absence of the Territory Administrator

Patient Care Oversight

  • Coordinate patient referrals and ensure interdisciplinary collaboration for optimal care delivery

  • Oversee the development, execution, and updates of individualized care plans

  • Evaluate quality of care and ensure patient needs are met through continuous assessment

  • Collaborate with clinicians and physicians regarding patient discharges or transfers

  • Review discharge assessments, Edit/Lock Discharge OASIS, and manage single-discipline discharges

  • Manage daily KPIs, workflow optimization, and escalation/complaint resolution

  • Support clinicians in implementing emergency care protocols

Personnel Support

  • Ensure all clinicians and contracted staff meet training and competency standards

  • Assign patient care responsibilities based on staff qualifications and competencies

  • Provide ongoing mentorship and guidance to field clinicians

Quality & Compliance

  • Lead and participate in QAPI (Quality Assessment and Performance Improvement) activities

  • Investigate incidents, identify trends, and implement corrective action plans

  • Prepare for and support accreditation/survey readiness

  • Conduct QA audits and ensure compliance with audit corrective actions

  • Manage HHCAHPS survey distribution via SHP

  • Oversee Emergency Preparedness Planning, activation, and event reporting


Qualifications

  • Bachelor’s degree in Nursing, Healthcare Administration, or related field (Master’s degree preferred)

  • Active clinical licensure (RN or applicable discipline)

  • Experience in home health leadership and familiarity with OASIS documentation

  • Experience with QAPI and survey readiness processes (preferred)

  • Strong knowledge of healthcare compliance, regulatory standards, and clinical quality measures

  • Excellent leadership, communication, and organizational skills

  • Ability to interpret clinical data and apply evidence-based practices

  • Proficiency in EHR systems and healthcare technologies
