RN Clinical Manager Home Health

We are looking for an experienced and highly motivated Clinical Manager to oversee and lead clinical operations in the field of home health.
 

Schedule: Full Time, 8am - 5pm

Will not have a team of clinicians reporting directly to them.

Responsibilities:

Supervision of Clinical Operations and Compliance: Responsible for overseeing day-to-day clinical and office operations. Available to support staff and patients during operating hours. Ensures that services are compliant with professional standards, agency policies, and state and federal regulations. Serves as a backup support for positions when needed: PCM, CSS. Assumes a leadership role in the absence of the Territory – Administrator.
Oversight of Patient Care: Coordinates referrals. Collaborates with an interdisciplinary team to coordinate patient care. Ensures the development, implementation, and updates of individualized care plans. Evaluates the quality of clinical care and ensures casual assessment of patient needs. Collaborates with clinicians and physicians regarding patient transfers or discharges. Reviews discharge assessment data, Edit/Lock Discharge OASIS, and discipline only discharge. Manages daily KPIs, workflow, and escalation/complaint calls. Provides guidance to professional clinicians on implementing Emergency Care Protocols.
Personnel Management: Ensures that clinicians, including contract staff, receive required training, competency, and supervision. Assigns patients and personnel, ensuring that assigned staff have demonstrated competence in the required tasks and services. Provides ongoing coaching and mentoring to field clinicians.
Quality and Compliance: Takes ownership of QAPI and participates in QA and performance improvement activities. Reviews and investigates incidents, analyzes trends, and implements corrective action plans when necessary. Manages accreditation/survey readiness, QA audits, and detailed corrective action plan audits. Oversees HHCAHPS distribution in SHP. Handles Emergency Preparedness Planning, activation reporting, ongoing event monitoring, and reporting.

 

Qualifications:

  • Hold a Bachelor's degree in nursing, healthcare administration, or a related field; a Master's degree is preferred.
  • Current nursing license or relevant clinical licensure.
  • Previous experience in HH leadership and knowledge of OASIS.
  • QAPI/Survey Readiness experience is a plus.
  • Demonstrated experience in a clinical management or similar role.
  • Thorough understanding of healthcare regulations, compliance standards, and quality assurance.
  • Excellent leadership, organizational, and communication skills.
  • Ability to analyze patient care data and implement evidence-based practices.
  • Proficiency in electronic health record (EHR) systems and healthcare technology.
