Registered Nurse (RN) Case Manager - Home Health
Responsibilities:

• Provide skilled nursing care to patients in need of professional nursing services based on physician's orders and under the supervision of the Clinical Services Supervisor.
• Efficiently and effectively manage each patient's care to achieve positive outcomes, including overseeing care provided by other nursing staff (LPN, HHA).
• May involve educating the patient's family and other members of the nursing team.
• Responsible for planning, delivering, and coordinating high-quality patient care.

Requirements:

• Must be a Licensed Registered Nurse (RN)
• Preferred: 2+ years of acute care nursing experience and/or previous experience in home healthcare
• Preferred: Experience with OASIS
• Preferred: Experience in med-surge and wound care
• Minimum requirement: Associate's Degree from an accredited college; Bachelor's degree highly desirable.
